Summary:
Pine Belt Mental Healthcare Resources has openings for Targeted Case Manager positions to work with a four-year System of Care children's grant that will cover Forrest, Jones, and Marion Counties. These positions will be based in Laurel and Columbia, MS. Position is M-F, 8a-5p; after hours and on-call duties required.


For 40 years, Pine Belt Mental Healthcare Resources has served individuals throughout the Pine Belt.  Through an extensive continuum of care, we strive to enable adults with serious mental illnesses, children with serious emotional disturbances, and individuals with an addiction or developmental disability to live, work, learn, and participate fully in their communities.  Our nine county service area includes Forrest, Jones, Lamar, Covington, Greene, Jeff Davis, Marion, Perry, and Wayne Counties.  
 


Requirements:
  • Master's degree in behavioral health field required
  • Must be a licensed social worker (LSW) or RN with two (2) years of work experience in mental health or have a Master's degree in a behavioral health field with either a professional clinical license or be eligible for a DMH Therapist Certification
  • Must possess a valid driver's license with a satisfactory driving record
  • Personal vehicle required to provide services in the community
  • Behavioral health care experience preferred
  • Ability to manage multiple projects and to complete assigned tasks on schedule
  • Ability to solve problems and resolve conflict in an efficient and timely manner
  • Ability to think independently to determine direction of work flow and work load
  • Fine motor skills for legible and accurate writing of reports, scheduling, daily correspondence and presentations either manually or orally
  • Position requires ability to sit for duration of several hours at a time and ability and mobility to access clients through home and community visits

  • Responsibilities:
  • Closely follows the progress of each youth and family enrolled in Crossver Xp and SOC, including follow up efforts after discharge.
  • Ensures all referrals to the Crossover Xp and SOC are reviewed and receive the appropriate level of care based on family and youth preference, EBP assessments, and life domain needs.
  • May provide services face-to-face or via telephone. Services may also be provided in the office or in the community.
