Summary:
A Community Support Specialist is needed to work on the PACT (Programs for Assertive Community Treatment) Team in Hattiesburg, MS. One (1) year of experience working with SMI population required. On-call duties required.
The PACT Program is a way of delivering a full range of services to people who have been diagnosed with a serious mental illness that seriously impairs their functioning in the community. The goal is to give consumers adequate community care and to help them have a life that isn't dominated by their mental illnesses. PACT teams provide services in the consumers' homes, where they work, and other community settings where problems occur or where support is needed. PACT Services are available 24 hours a day.
For 40 years, Pine Belt Mental Healthcare Resources has served individuals throughout the Pine Belt. Through an extensive continuum of care, we strive to enable adults with serious mental illnesses, children with serious emotional disturbances, and individuals with an addiction or developmental disability to live, work, learn, and participate fully in their communities. Our nine county service area includes Forrest, Jones, Lamar, Covington, Greene, Jeff Davis, Marion, Perry, and Wayne Counties.
Requirements:
Education/License:
Bachelor's degree in behavioral health required
Must be able to obtain DMH Case Management certification within 60 days of hire
Must possess a valid Mississippi Drivers License with a satisfactory driving record
Must have reliable transportation to provide services in the community
Work Experience/Skills:
Behavioral health experience preferred
Excellent communication skills required
Ability to solve problems and resolve conflict in an efficient and timely manner
Sedentary work endurance ability
Fine motor skills for legible and accurate writing of reports, daily correspondence and presentation, either manually or orally
Independent work ability
Responsibilities:
Provide crisis intervention, brief therapy services, and on-call services.
Partners with community agencies to provide services.
Provides all Community Support Specialist services as defined by Department of Mental Health Operational Standards.
Provides community referrals and outreach services.
Provides assistance with ADLs, housing, family life, employment, benefits, finances, medication management, access to health care, co-occurring disorders, counseling, and education.
