Wraparound Facilitator-System of Care Grant

Summary:
Pine Belt Mental Healthcare Resources has openings for Wraparound Facilitators to provide services to youth in Jones and Marion Counties as part of a four-year grant. This position is responsible for engaging families, youth, and formal/informal supports key to each family/youth in the development of youth and family teams that address their needs for intensive integrated care.

Work hours are typically M-F, 8-5, with some after hours and on-call duties required. This position has a projected hire date of October 1, 2017.Positions are located in Laurel and Columbia, MS.

Requirements:
Education/License:

  • Bachelor's degree in behavioral health field required
  • Must be able to obtain DMH case management certification within 90 days of hire
  • Must possess valid driver's license with satisfactory driving record
  • Must possess personal transportation to provide services in the community.

    Work Experience/Skills:

  • Behavioral health experience preferred
  • Ability to solve problems and resolve conflict in an efficient and timely manner
  • Ability to speak in front of groups and to communicate effectively through verbal and written communication methods
  • Ability to think independently to determine direction of work flow and work load

  • Responsibilities:
  • Communicates appropriately with outside referrals regarding consumer participation and progress in WRAP process.
  • Completes all documentation thoroughly, accurately, and within time frame required by using Electronic Medical Records (EMR). Provides written documentation of consumer's progress via progress notes.
  • Completes Strengths-based Assessments, Family Stories/timelines, and Family Vision on each youth/family served.
  • Facilitates the creation of a child and family team for the purpose of developing a single plan of care to address the needs of children and youth with complex mental health challenges and their families.
  • Facilitates the identification of underlying needs for each youth/family served.
